We here at Diablo Valley Oncology / Hematology Medical Group recognize that your time is valuable. In an effort to make the referral process as easy as possible we have created this form for you.

Someone will be in contact with you and (or) the patient as soon as possible.

Desktop Shortcut for your PC

• Open internet explore
• Enter in the URL for our referral form
• http://diablovalleyoncology.md/refer
• Highlight the URL in the browser window
• Right-click
• Select “create a desktop shortcut”
• On your desktop you will find an icon for the form
• To rename the icon: right click and choose “rename”
• Change to “Diablo Valley Oncology Referral” (or any other name that makes it easy for you!)

From now on, any time you wish to refer a patient to us all you have to do is click on that icon and it will take you directly to the form!
