Patient Portal Information for Diablo Valley Oncology & Hematology

The Patient Portal can be used to:

    1. Communicate with our office by sending us a message using the feature “Ask a Question” and select a topic to add your message.
      • “Ask a Question” regarding appointment information, prescriptions, billing, status of an authorization from your insurance, new patient-related questions, request a note from your doctor, request your medical records or a copy of your lab results, or you can ask to receive our DVO newsletter.
    2. View your past appointments, demographic information, medications, messages, and your requested lab results.
DISCLAIMER: If you think you have a medical emergency, please dial 911 or your local emergency number for immediate assistance.  If you are experiencing symptoms, please call our office at (925) 677-5041.  Do not rely on electronic communications for urgent medical needs.  The portal should not be used in place of consultation with appropriate healthcare professionals, and is not designed to facilitate or assist with medical emergencies.

To register for the Patient Portal:

Please call our office at (925) 677-5041, or submit your request through our website at www.dvohmg.com. Once your request is received, DVO staff will send you an email invitation to create your patient account by clicking on the enclosed “Sign Up!” link. You will be prompted to create your username, password, and security questions/answers. For questions regarding the Patient Portal, please call our office and ask to speak with our Medical Records Department.

To access the Patient Portal once enrolled:

Go to our website, and then click on “Patient Portal” to go to the Medfusion log in page.

To send our office a message, click on “Ask a Question” on the “Home” page:

Next, make the selection regarding what your question is about:

After making your question select, the next screen will display your question category and provide a text box (shown below) to enter your question for our office. Then, click on “Continue” in the lower right corner.

The next screen will display the information that you are about to submit. If you need to make any changes, click on the “Back” button. If you are satisfied with your entry, click on “Submit Question.”

To view notifications from our office, click on the “Messages” icon on the “Home” screen.  This feature functions as your inbox.  You will receive an email and notification after each appointment confirming your health record data.     _____________________________________________________________________________________________________________________     To view your past appointments, demographic information, medications, and message attachments, click on the “Health Record” icon on the “Home” screen.      

**NOTE:  Imaging reports are not available on the portal at this time.

Lab results are available upon request.

PATIENT PORTAL TROUBLESHOOTING

I forgot my user ID.
      1. Use the “forgot your user ID?” link on the portal login page.
      2. Enter your email address.
      3. Confirm your date of birth.
      4. Answer your secret question.
      5. Your User ID will be sent to your email address from Medfusion.
I forgot my password.
      1. Use the “forgot your password?” link on the portal login page.
      2. Enter your user ID you created.
      3. Answer your security question and develop a new password.
      4. Open your email and click the “confirm my password” link.
      5. Confirm your password.
I tried that, but don’t remember my secret question. Help! Call our practice and we’d be happy to email you a direct link to reset your password. However, DVO cannot change your security questions or answers. Only patients can change their secret questions under their “My account” settings once logged into their portal account. I don’t have access to the email address I used when I set-up my portal account. Call our practice and we’d be happy to update your email address in Mosaiq and Medfusion, and assist you with your user ID and/or password reset. What are the requirements for my password? Your password must be at least 6 characters, must contain at least one number OR an upper case letter, and should not contain any special characters. I’ve been locked out because of too many password attempts. Please wait 15 minutes and try again. We set a maximum number to ensure the security of your account. What browser should I use? Internet Explorer, Mozilla Firefox, Chrome and Safari are all supported. Be sure you’re using a current version of your web browser for optimal performance.
